Prescription Formulary Guide

Minnesota's nonprofit health plans strive to provide consumers with the most effective, affordable and highest quality prescription drugs available. The list of prescription drugs covered through your health plan is called a formulary.

The MN Outpatient Drug Formulary Guide lists the most commonly prescribed drugs, by category and health plan. The Guide is now published by Minnesota Physician Publishing and is distributed to physicians through MN Physician Magazine. Pharmacies in the state received the Guide through the Minnesota Pharmacy Association.

Note: Our Formulary Guide lists drugs for the most commonly prescribed categories for outpatient medical care for participating Minnesota health plans. It is not a complete list. The list was last updated July 2006, and does not reflect changes made since then. Drugs are listed by health plan. In addition, this guide contains the following designations: *preferred formulary drug, PA–Prior authorization required for this drug, ST-step therapy and QL- quantity limits.
