Prescription Drug Guide
Minnesota's nonprofit health plans strive to provide consumers with the most effective, affordable and highest quality prescription drugs available. The list of prescription drugs covered through your health plan is called a formulary.
The MN Outpatient Drug Formulary Guide lists the most commonly prescribed drugs, by category and health plan. The Guide is published by Minnesota Physician Publishing and is distributed to physicians through MN Physician Magazine.

The list was last updated July 2007, and does not reflect changes made since then. Drugs are listed by health plan. In addition, this guide contains the following designations: *preferred formulary drug, PA–Prior authorization required for this drug, ST-step therapy and QL- quantity limits. This site is provided as an informational service to the public, health plan members and physicians. The information provided does not include all of the drugs on each health plan's formulary, and because these formularies change periodically, may not contain the most recent information. Contact the health plan directly for the most current, accurate information.
